DEVELOPMENTAL SERVICES SUPPORT COORDINATORGoochland Powhatan Community Services is seeking a Full-Time Developmental Services Case Manager.This position is responsible for providing support coordination to support children and adults with Intellectual Disabilities (ID) and/or Developmental Disabilities (DD). Support Coordination includes conducting assessments, developing person-centered plans, and assisting eligible consumers in accessing the most appropriate services to meet their individual needs. This is accomplished through linking, monitoring, documenting, coordinating resources, and advocating for effective and responsive service provisions.Bachelor's degree in human services and experience with ID/DD population is required.The GPCS application is required and available at www.gpcsb.org under "Employment." Salary range is $50,000 - $69,031 based on education and experience.Applications must be submitted by 4:30 pm on Friday March 1, 2024.GPCS is an Equal Opportunity Employer.
